  • Surgical Planning and Access: Radiographic identification of the radial tuberosity with image intensifier. Skin marking with dermographic pen. A fascia opening without damaging muscle, with strict hemostasis and without cuff.
  • Neurovascular Dissection and Protection: Dissection of the brachioradialis muscle along the direction of the fibers. Ligation of the recurrent radial artery branch. Careful identification and retraction of the deep branch of the radial nerve.
  • Preparation of the Bone and Tendon Site: Identification of the radial tuberosity and the remaining tendon. Manual positioning of retractors. Drilling of the cortices with guide wire and drill according to tendon thickness. Remodeling of the distal tendon end and cleaning of the insertion site.
  • Suture and Fixation with TightRope: Suture with Fiberloop in a braided pattern. Determination of the drill diameter (7mm). Passage of the endo-button and precautions to avoid compression of soft structures. Progressive introduction of the tendon into the tunnel and tightening of the knot with proper approximation.
  • Verification and Closure: Anatomical evaluation of positioning with X-ray.Attention to the proper construction of the tunnel. Preservation of muscle and neurological structures. Anatomical closure of the fascia and aesthetic intradermal suture of the skin.
