• Evaluation and Planning: Palpation of the limit of brachioradialis and use of fluoroscopy to locate the tuberosity of the radius. Planned skin access to avoid muscle injury and precise dissection of the fascia.
• Dissection and Hemostasis: Ligation of a branch of the radial artery, with effective control of bleeding, eliminating the need for a pneumatic cuff.
• Exposure of the Reinsertion Site: Visualization of the tuberosity of the radius with full supination of the forearm, exposing the site of detachment of the biceps.
• Tendon Preparation and Suturing: Triangular modeling of the tendon tip to facilitate insertion. Suturing with Arthrex material, slightly tight for stability.
• Bone Orifice and Fixation: Measurement of tendon thickness, drilling the cortices with a guide wire and drill, creating a 7 mm hole in the first cortex.
• Reinsertion with TightRope: Introduction and fixation of the already sutured tendon with the TightRope device on the opposite side of the radius.
• Verification and Closure: X-ray confirms the orifice. Anatomical and delicate access, without muscle injuries, with a favorable aesthetic result.
