Dislocation of the proximal humerus with fracture of the greater tuberosity in a high-performance athlete. I opted for anatomical reduction and stable fixation of the fracture through the deltopectoral access, which was performed without muscle cuts; respecting the interval between the pectoral muscle and the deltoid muscle and preserving the cephalic vein. For fracture stabilization, a mini-fragment plate with long locking screws and a rotator cuff ligature was used. Immediate rehabilitation with isometry of the deltoid with abduction and flexion up to 45 degrees and pendular exercises.
I had the opportunity to treat this rare association of lesions for the second time; clavicle fracture associated with posterior acromioclavicular dislocation (Rockwood IV); Different from the first case I operated on; In this one, I did the procedure through a less invasive technique with the fixation of the clavicle made by a bridge locking plate; and fixation of theacromioclavicular joint performed with a tighRope like device.
Like this; The coracoclavicular fixation offered vertical stability, while the use of a metal anchor on the lateral aspect of the clavicle allowed the reconstruction of the acromioclavicular ligaments, offering horizontal stabilization.

This video is a step-by-step demonstration of the surgical treatment of distal biceps tendon rupture through conventional open technique and anatomical dissection of adjacent structures; Connecting all branches of the recurrent radial artery to avoid hematoma in the immediate postoperative period. With anatomical dissection and wide visualization of the radial tuberosity, we guarantee a correct reconstruction and avoid fractures as complications.

The art of bone reconstruction for tibial plateau fracture
The Shatzker and Kfuri classification directs us to the best therapeutic approach for tibial plateau fractures, taking into account the integrity of the cortical ring. In large subsidences (collapses), even though they are mainly posterolateral; as long as there is no deviation of the cortical ring; I use the anterolateral approach and perform an incomplete osteotomy of this plateau to lift the subsidence. In the future I will describe this "incomplete osteotomy".
In this case, I used a sintetic structured bone graft block to support this subsidence.

Nos grandes afundamentos (colapsos), mesmo sendo principalmente postero-laterais; desde que nao exista desvio do anel cortical; utilizo a abordagem antero-lateral e faço uma osteotomia incompleta deste planalto para levantamento do afundamento. Futuramente irei descrever esta "osteotomia incompleta".
