Dislocation of the proximal humerus with fracture of the greater tuberosity in a high-performance athlete.  I opted for anatomical reduction and stable fixation of the fracture through the deltopectoral access, which was performed without muscle cuts;  respecting the interval between the pectoral muscle and the deltoid muscle and preserving the cephalic vein.  For fracture stabilization, a mini-fragment plate with long locking screws and a rotator cuff ligature was used.  Immediate rehabilitation with isometry of the deltoid with abduction and flexion up to 45 degrees and pendular exercises. ...

I had the opportunity to treat this rare association of lesions for the second time; clavicle fracture associated with posterior acromioclavicular dislocation (Rockwood IV); Different from the first case I operated on; In this one, I did the procedure through a less invasive technique with the fixation of the clavicle made by a bridge locking plate; and fixation of theacromioclavicular joint performed with a tighRope like device.
Like this; The coracoclavicular fixation offered vertical stability, while the use of a metal anchor on the lateral aspect of the clavicle allowed the reconstruction of the acromioclavicular ligaments, offering horizontal stabilization.
